Tom Mansfield was no stranger to pre-workout supplements, but a miscalculation when weighing his caffeine powder caused him to go into cardiac arrest.

Tom Mansfield prioritized three things above all else: his family, his physical fitness, and the health of his clients. Before exercising on Jan. 5, 2021, however, the personal trainer made one fateful mistake. After accidentally measuring out far too much pre-workout caffeine powder, he drank the equivalent of almost 200 cups of coffee — this staggering overload of caffeine caused the fitness trainer to die within the hour.

The 29-year-old father of two had purchased a bag of caffeine powder from Blackburn Distributions, a British sports supplements company. There was no measuring scoop included with the product. Mansfield was at his Colwyn Bay home in Wales when he attempted to weigh out the proper amount of caffeine powder. The recommended dose was between 60 and 300 milligrams, but Mansfield’s kitchen scale started at two grams. While trying to convert the measurements, Mansfield mistakenly added a staggering five grams of the powder to his drink.

A legal inquest into his death concluded that Mansfield had caffeine levels of 392 milligrams per liter of blood in his system. Caffeine levels after a typical cup of coffee average two to four milligrams per liter of blood. Mansfield had so thoroughly overburdened his heart that his legal cause of death was caffeine toxicity.

Ambulance response times in England were the worst on record across all major categories in December 2022, while people waiting 12 hours or more to be admitted to A&E reached an all-time high. The average response time for people requiring an ambulance for a stroke, severe burns or chest pain was 93 minutes, five times the operation target of 18 minutes. The higher-priority “life threatening” category was also badly affected in December, with ambulance crews taking almost 11 minutes on average to reach patients – including those suspected of having a heart attack.

I personally think that caffeine powder is dangerous and should be banned right across the board..Australians who add pure caffeine to their protein shake or 'dry scoop' their pre-workout protein mix and eat it raw are putting their lives at risk, a top toxicologist has warned.

The sale of pure caffeine for personal use was banned in Australia in 2019 after a report found a single teaspoon of the powder - equivalent to 50 cups of coffee - contained 5,000 milligrams of the drug and could cause cardiac arrest. 
 

Party-going teens are also snorting caffeine available online and linked to OD deaths..

Lachlan Foote Above - A father has shared his grief over finding out the cause of his son’s unexpected death, taking to Facebook to warn others of the widely available ingredient that killed 21-year-old Lachlan Foote a day before his birthday. After celebrating New Year’s Eve, Mr Foote arrived home and made a protein shake before messaging his friends complaining of the taste. "I think my protein powder has gone off. Just made an anti hangover / workout shake and it tasted awful," he wrote just after 2am. Hours later Lachlan passed out on the bathroom floor and never recovered. The Foote family were grieving the loss of Lachlan while denying rumours that the aspiring musician’s sudden death was due to a drug overdose. “I think the rumour about Lachlan's death probably started fairly innocently because it happened on New Year's Eve, people make assumptions,” Lachlan’s father, Nigel Foote, said. Pure caffeine powder is widely available and sold as a dietary supplement, with one teaspoon equivalent to up to 50 cups of coffee. A simple miscalculation could result in almost certain death.. A drunk individual could easily consume more than is the safe level...

The paramedics employed a defibrillator on Mansfield to subdue his “grossly abnormal” heartbeat. He went into cardiac arrest and was rushed to Glan Clwyd Hospital — where doctors officially pronounced him dead at 4:00 p.m. 

Widowed and heartbroken, Suzannah Mansfield said her husband was a “really healthy” person. She explained that he had initially only taken a sip of his drink before he chugged the rest of it and the fatal side effects reared their head. “He was my whole life. I’ll never get over this,” she said.

Senior Coroner John Gittins testified that Mansfield’s death was ruled a “misadventure.” He said Mansfield was “likely aiming for a mid-range serving” of the recommended dose of caffeine powder when “he got his maths wrong.” The coroner also stated that Mansfield would likely still be alive if Blackburn Distributions had provided a measuring scoop with the powder.

A statement from company director Ben Blackburn that was read at the inquest said that the powder, which is used to boost energy levels, should always be weighed to two decimal places in milligrams. Blackburn added that the recommended dosage is between 60 and 300 milligrams once or twice daily.

Blackburn Distributions has begun adding measuring scoops to their powders. The company also claimed that they will include more detailed instructions and clearly highlight any side effects on their packaging. While the inquest heard that the warning labels and instructions on the packet Mansfield used were not in breach of regulations, Suzannah Mansfield has naturally found no solace in the proceedings that followed his death. Now a single mother, she’s left to raise her children, Tommy and Millie, alone. “I’m missing you so much and trying so hard for the kids [to] keep it together,” she wrote on her social media pages. “I love you more than I could love anyone, I’ll always love you.”

A SCHOOLBOY was stung to death by bees after a hive fell on his head as he took shelter under a tree during a storm.

Felipe Silva do Rosario, 13, was killed when strong winds caused the freak accident in Santos, Brazil.

The beehive was battered by the weather and fell from the tree directly on to the young boy's head. Angry and disturbed bees then attacked Felipe, he was stung repeatedly causing severe injuries. Disorientated by the blow to his head, he tried to get up but fell over again, hitting his head once more on a piece of wood. The windstorm was caused by an extratropical cyclone passing through the area on the 22nd May 2021. Felipe was out in the storm after leaving home to check on his grandmother's animals. His last words to her was: "Gran, I'm just going to the hen-house to see if everything is all right with the hens." Felipe was accompanied by his seven-year-old brother who witnessed the incident, but was unable to help after being beaten back by the swarm of bees. The traumatised brother ran back to explain what had happened to his grandmother, who then called for an ambulance.

A man drowned after jumping into the water after escaping from a swarm of aggressive bees in Brazil..

Donizete Bezerra da Silva, 58, was attacked by “killer bees” when he was mooring his canoe to a tree on a farm in the municipality of Catalao, state of Goiás, in central Brazil. The man didn’t realize there was a swarm of bees hidden in the tree where he moored his boat. According to police spokesman Lieutenant Robson Julio de Souza, the victim had apparently tried to tie down his canoe very securely causing the tree to shake. Unknown to da Silva he had accidentally disturbed a hive of bees..

Da Silva instinctively jumped into the water with a friend who he was with at the time in order to escape the swarm of Africanized bees that are known as “killer bees” for their aggressiveness.

The local Fire Department confirmed that the 30-year-old man was fishing with friends last Saturday (30 October 2021) when they were suddenly attacked by the swarm of bees. Da Silva quickly entered the water to shield himself from the angry bees. Unfortunately Da Silva got into difficulty and drowned while fighting off the bees. To add honey to the wound he was then set upon by a school of ravenous Pirannas who devoured his face and other parts of his body..

Everyone knows that piranha are carnivorous - but they're not entirely so. An analysis of the stomach contents for fish caught in the Pantanal shows a surprisingly high level of vegetable matter - particularly for P. nattereri and S. spilopleura, where appears to be a significant part of their diet. The non-vegetable portion of their diet consists of insects, crustaceans, fish eggs, scales/fins, and small vertebrates and fish. Piranha appear to be opportunistic scavengers more than mindless assassins - often exhibiting "grazing" behaviour where they quickly nip at other fish passing by (including other piranha). This is non-lethal to the prey. They also scavenge the carcasses of other dead animals in the water. Piranhas prefer to approach their prey from behind.

The seasonal rise and fall of most waters where piranhas live is also directly and indirectly responsible for numerous piranha deaths. During the dry season many piranhas of all sizes become trapped in desiccating floodplain lakes and ponds cut off from the rivers. Most either die or become food items for predators and scavengers such as fish-eating mammals, wading birds, jacaré, water snakes and vultures. Piranha hunt, but are opportunistic - targeting easy prey which appear old, weak, wounded or exhibiting signs of distress. Hence, the common triggers for attacks are splashing and the scent of blood. Piranha are also food for other animals such as Giant Otters, amazon dolphins and numerous species of birds. Piranha swim around in shoals of 20 to 100 as a means to protect themselves from predators.. Piranha attacks on people are rare - but their fearsome nature means they can quickly make headlines worldwide when they happen.

The horror incident happened on Saturday in a rural area of Brasilandia de Minas, a municipality in the state of Minas Gerais in Brazil around 600 miles north-west of Rio de Janeiro.

The most dangerous period for attacks is usually the dry season. This is where the piranha are concentrated into smaller areas - and less food in the environment makes them more aggressive. This is less of an issue in major rivers where they are free to move further downstream - but lagoons and oxbow lakes can be especially dangerous. As a result of attacks, wire mesh nets and public address systems have been installed around some popular river beaches. However, rather than mass attacks, random attacks where a single piranha takes a bite out of a passing swimmer or person wading through the water (i.e. piranhas exhibiting their natural "grazing" behaviour) does occasionally occur. Splashing tends to make piranhas more likely to attack, and children are often attacked for this reason.

Specialist divers found the man’s body 4 metres from shore the next day and found that the carnivorous fish has disfigured the man’s face and other parts of his body. The family of the deceased have since given permission to bury his body after investigators ruled a tragic accident had occurred and there was no sign of any crime.

An English firm called Brazil Land Cattle and Packing Company used to own a large area of Brasilandia de Minas but the lands were abandoned during the Second World War and later expropriated by President Getulio Vargas.

The municipality is now home to around 17,000 people and boasts a tropical climate with temperatures ranging from 15 to 38 degrees Celsius.

Around 30 species of piranhas are said to inhabit South America’s lakes and rivers. 

  • If swimming in rivers with piranhas, the breaststroke is recommended since this causes less noise and stirring in the water. In closed ponds and lagoons it is recommended not to swim at all.

A man who was accidentally shot by his dog in a tragic hunting accident gone wrong has been identified as Kansas plumber Joseph Austin Smith as friends remembered the hunter as a “loving goofball” who made them laugh and smile. Smith, 30, was in the front passenger seat of a pickup truck when the pooch stepped on the hunting rifle in the backseat. Smith was struck in the back and killed instantly, from the explosive and powerful cartridge.

Smith was a plumber at Browns Plumbing Services in Wichita, the company’s owner, Chris Brown, said. “Joe is absolutely the most unique individual I have had the pleasure of meeting. He was loved by all of us from all walks of life and loved the same,” Brown wrote in a GoFundMe to help Smith's family cover funeral costs. “He made us laugh every morning whether it was at him or because of him he always made sure to be a positive light at my company. No matter how bad or stressful my or our day was, Joe always had something to lift our spirits.” “Joseph Smith was a truly amazing man. He was kind, funny, smart, and very loving. Every time he walked through the door, he would make someone smile,” wrote friend and co-worker Jessica. “Either from being a goofball or just his natural presence making everything better. It wasn’t hard to love Joe because he always made you feel like you were one of a kind and never left a conversation without letting you know he loved you.”

The gun also belonged to his friend, the sheriff’s office said. The friends had been hunting doe, and the back seat had hunting gear and a rifle. It’s unclear if the truck was moving at the time. The Sumner County Sheriff’s Office determined that a dog that belonged to the owner of the truck had caused the rifle to discharge,

The unnamed man, from Geldrop, was treated by paramedics on the scene but his injuries were too severe and he died, local media Omroep Brabant reported.  Police said he died when a ‘metal ball in use at a sporting event’ landed on him, with reports adding somehting had gone ‘wrong during the hammer throw’.

A man was killed after getting struck by a 22 pound metal ball being used in a competition at the Open Highland Games in the Netherlands.

A witness said: ‘The pendulum gave way, we saw the ball go over the hedge and then we heard a woman screaming very loudly.’ They said the man was walking past and was not a spectator, adding: ‘The victim walked behind a hedge in one of the gardens at Geldrop Castle.  ‘He was not on the site where the event was held. He didn’t see the ball coming at all.’

Emergency services rushed to the scene while event organizers attempted CPR, but the victim — who was not publicly identified — could not be saved. The sporting event was then cancelled. The throwing event is a traditional part of the Highland games originating in Scotland and involves a competitor swinging a “hammer” around their head before letting go and seeing how far it reaches. “We are flabbergasted that something like this can happen,” said organizer Adri Geerts, chairman of the Foundation Village Marketing Geldrop-Mierlo.

“You don’t expect it, it’s the biggest disaster that can happen to all of us. We’re devastated.” According to witnesses, the athlete who threw the ball was one of the top competitors in the sport. “The boy who swung the ball was completely upset,” one person said. “He was not a rookie, but really a top player, who had already participated in big competitions before.” Geerts said all the competitors were distraught over the death and were being provided with services. He said he has not spoken to the athlete who threw the hammer. “I haven’t spoken to him myself, but he’s completely beside himself, this should never have happened to him,” Geerts said. “They are players who practice this game internationally at a high level and with a lot of experience and are extremely experienced, but then apparently an accident can happen,” he added. Deputy Mayor Hans van de Laar released a statement offering condolences to the family. “We are all very affected by this terrible accident. Our condolences go out to the bereaved families,” Laar said. Netherland police are investigating. An air ambulance was called to the scene and police ordered everyone to leave the area after the man was struck.

The vehicle surged backwards and trapped him under its wheels. The victim's partner 56 year old Iris Fortner had desperately tried to stop the car before it backed into Campbell. She was in the driver's seat but was unable to prevent the accident. Florida Highway Patrol officials said the accident took place outside the couple's home in Cantonment in Florida's Panhandle. Campbell and Fortner were backing into their driveway when he got out to open their gates. When Fortner opened her door to see where Campbell was standing police said their large boxer bull dog jumped into the car and pressed the accelerator. The car was engaged in reverse gear and shot backwards hitting Campbell who was pronounced dead at the scene by Escambia County Emergency Medical Services. Fortner has not been charged.

After Yelchin failed to arrive at a rehearsal, he was found by friends after midnight on June 19, 2016, pinned between his Jeep Grand Cherokee and a brick pillar gate post outside his house in Studio City, Los Angeles, the victim of what was described as a "freak accident". As Yelchin got out of his car and went to check his locked gate and mail, the vehicle apparently rolled back down his driveway, which was on a steep incline, and trapped him against the pillar and a security fence. Yelchin was pronounced dead at the scene at the age of 27, the Los Angeles County Coroner's office identified the cause of death as "blunt traumatic asphyxia"  and stated that there were "no obvious suspicious circumstances involved".

Yelchin enjoyed playing guitar, which he said gave him fulfillment, and was a fan of acoustic blues music. He once played for a punk band called The Hammerheads. He was also interested in photography. In January 2018, an exhibit of Yelchin's photography entitled Anton Yelchin: Provocative Beauty was displayed at New York City's De Buck Gallery. Yelchin's photography has since been critically acclaimed by various magazines around the world as exhibits continue.

Following Yelchin's death, the producers of Star Trek stated that the role of Chekov would not be recast and that the character would be written out of any subsequent Star Trek films. Star Trek Beyond was dedicated to Yelchin and Leonard Nimoy, who also died between the release of Star Trek Into Darkness and Star Trek Beyond. Smurfs: The Lost Village was dedicated to Yelchin, as he had provided the voice of Clumsy Smurf in the live-action Smurfs films.

Fiat Chrysler Automobiles (FCA), the manufacturer of the Grand Cherokee, was aware of 2014 and 2015 models having a high rate of rollaway incidents due to a gearshift design that could make it difficult for the driver to determine whether the vehicle was in park or still in gear. FCA had already recalled all 2014–15 Grand Cherokees for this concern in April 2016, but the software patch to repair the vehicles did not reach dealers until the week of Yelchin's death. Following his death, FCA accelerated the recall campaign and took steps to get the affected Jeeps repaired more quickly than originally planned.

On August 1, 2016, Yelchin's parents announced through their attorney that they were planning to file a wrongful death lawsuit against Fiat Chrysler. The dealership from which Yelchin purchased the vehicle stated that he was responsible for his own death, because he had allegedly "misused" and "modified" the vehicle. The dealer also asked to be removed from the lawsuit. On March 22, 2018, it was announced that Yelchin's family and Fiat Chrysler had confidentially settled out of court.

An eight-year-old boy was found dead in his bedroom after inhaling a helium balloon from his own birthday party. Luke Ramone Harper, from Dublin, suffered fatal brain injuries induced by helium after he placed a balloon in the shape of the number 8 that had been bought for his birthday party a week earlier over his head.

The young boy was pronounced dead at Temple Street Children’s Hospital in Dublin on April 2nd 2021..His mother, Hilary McSweeney, told an inquest at Dublin District Coroner’s Court that Luke had been playing upstairs the previous afternoon and watching YouTube videos. Ms McSweeney said her son, who had autism and had been diagnosed with ADHD, had not responded when she called him and his sister, Ella, to come down for dinner shortly before 5pm. She described discovering him lying face down on the ground upstairs in an unresponsive state with a balloon completely over his head. Ms McSweeney said the balloon had been floating in the sitting room for the previous week after his birthday party and was partially deflated. She said her son’s body was limp as she pulled the balloon off his face and rushed him outside into the back garden to get some fresh air.

The inquest heard she and Luke’s father, Martin Harper, gave their son chest compressions while they waited for emergency services to arrive. Ms McSweeney said she knew that Luke was “in trouble” by the amount of time paramedics had spent working on him in the garden before taking him by ambulance to hospital. While in hospital, she heard that he had suffered a cardiac arrest. “It was obvious his brain was very badly damaged and he was on life support,” Ms McSweeney recalled. “I knew Luke was dead at that point and the machine was keeping him alive.”

After being informed by doctors the following day that it was time to say goodbye, Ms McSweeney said they took mementos of Luke through his hair, handprint and footprint. She recounted how she lay on her son’s bed with his favourite blanket and teddy bear and surrounded by his family as he passed away. He was formally pronounced dead at 3.52pm on April 2, 2021. “I felt numb, heartbroken and in a pain you can’t imagine,” she told the inquest. Describing Luke, she said he was an excellent son who was “full of confidence and full of loving”. In response to queries from the coroner, Cróna Gallagher, Ms McSweeney said her son had always had helium balloons for his birthday parties but there had never been a similar incident or any attempt to put a balloon over his head. “It was a complete freak accident,” she remarked.

Mr Harper said his son would have been totally unaware that helium was sometimes inhaled by older children because of the way it changes their voice. He said the family believed that Luke was just trying to reinflate the balloon and he “blacked out”. A post-mortem found Luke died as a result of a lack of oxygen to the brain due to a cardiac arrest which had been caused by the inhalation of helium from a partially-deflated balloon. Dr Gallagher said he would have suffered rapid and severe brain damage in what was “an insurmountable injury.” The coroner said Luke was likely to have lost consciousness very quickly after inhaling the helium as there was no evidence that he had tried to take the balloon off his head or seek help from others. Dr Gallagher said the evidence indicated it was the gas and not the balloon which was responsible for the tragedy. The coroner said she was reluctant to make any recommendations in the case as it would suggest the accident might have been predictable.

Police in Catalonia are investigating the death of a man who is thought to have become trapped inside a large dinosaur statue while trying to retrieve his mobile phone. Officers were called to the statue in Santa Coloma de Gramenet, a satellite town of Barcelona, after a man and his son noticed something inside the papier-mache stegosaurus. A spokeswoman for the regional police force, the Mossos d’Esquadra, said the death of the 39-year-old man was not being treated as suspicious. “A father and son noticed that there was something inside and raised the alarm,” she said. “We found the body of a man inside the leg of this dinosaur statue. It’s an accidental death; there was no violence. This person got inside the statue’s leg and got trapped. It looks as though he was trying to retrieve a mobile phone, which he’d dropped. It looks like he entered the statue head first and couldn’t get out.

A spokeswoman said officers had called in firefighters to help cut into the statue so the body of the man could be removed. “We’re still waiting for the autopsy results, so we don’t know how long he was in there, but it seems he was there for a couple of days,” she added. According to local media, the dead man had been reported missing by his family a few hours before his body was discovered.

Investigators believe grocer Sergio Millan, 59, died after a metal plate weighing over a ton flew through the window of the flat above his and hit the floor before bringing down his ceiling below on top of him crushing him to death..

The blast took place on the premises of Iqoxe, according to a woman who answered the phone at the company and who quickly hung up the phone without giving her name. According to the company's website, Iqoxe is Spain's sole producer of ethylene oxide, a chemical compound used for making detergents, solvents and other products, as well as glycol, one of the main raw materials in the production of plastics. Local residents posted videos showing the aftermath of the blast, with flames and a big column of black smoke emerging from an area dotted with big industrial tanks. Fire departments sent 24 brigades to the area, the regional emergency service said. Some local residents told Tarragona Radio that the blast could be heard from miles away.

However an alert was lifted after firefighters confirmed the plume of smoke caused by the blasts and resulting fire was not toxic.

The fire was brought under control overnight. 

Catalan president Quim Torra said late last nigh: “I would like to send my condolences to the family of the person who died as a result of this accident.

“We would also like to send a message of tranquility to the population because no toxic agent has been detected in the air.”

Tarragona Town Hall has announced two days of mourning.

Tarragona is located 115 kilometers (71 miles) southwest of Barcelona, the regional capital of Spain's Catalonia region. Since 2015, the city has housed a 1,200-hectare (2,965-acre) “chemical hub,” ChemMed, that was described as the largest of its kind in southern Europe. The government said Prime Minister Pedro Sánchez had been in contact with the regional president of Catalonia, Quim Torra to inquire about the blast and offer any help needed.

Rescue teams with search dogs found the body of the second victim, a senior staff member at the factory, in the early hours of Wednesday. The search had been called off overnight due to poor visibility and dangerous conditions.

The metal plate linked to Mr Millan’s death is believed to be the top of the reactor where ethylene oxide was being produced to be used in products including detergents. Officials have said they believe the explosion occurred inside the reactor. The dead man lived with his wife just under two miles from the plant in an area on the outskirts of Tarragona called Torreforta. A friend of Sergio's said: Today is a sad day, knowing that neighbour and friend Sergio Millan will no longer walk his dog through the streets of our neighbourhood. Teddy Balkind, a 10th grader at the St. Luke’s School in New Canaan, was fatally injured Thursday night during a private-school matchup. Thursday January 6th, 2022.. Teddy collided with another player on the ice on 6th January 2022 and was taken to hospital. The medical examiner announced the results of the following investigation on Monday 10th 2022. The hockey player was in tenth grade at St Luke’s School in New Canaan. Sam Brande (pictured) has started a petition in the wake of the death of his friend, Teddy Balkind. Balkind died Jan. 6, 2022 in a hockey-related accident. The petition calls for the mandating of protective neck guards in hockey.

A 72-year-old Kentucky grandmother who was killed by a falling Denny's sign had just picked up her dying husband from the hospital when tragedy struck. Lillian Mae Curtis, her husband Lloyd Eugene Curtis Sr., 77, and their daughter Mary Graham, 58, were believed to be stopping for food on Thursday afternoon 19th January 2023 when the 2,600lb + sign fell onto their car amid  moderate 50 mph gusts in Elizabethtown.

Lillian, who was sitting in the back seat of the car, was immediately crushed by the falling sign. The grandmother was trapped in the vehicle and had to be extracted from the vehicle by emergency workers. 

Her granddaughter, Amy Nichols, told WDRB News her head wound had been 'catastrophic' and 'inoperable.' 

Howard told KVUE that her grandmother's death had been 'absolutely instantaneous and that there was no way her body could have felt any pain.'

Lloyd had been rushed to hospital with fluid in his lungs and was waiting to be transferred back to the Jewish Hospital.  

Mary Graham, 58, who had been driving, sustained five broken ribs and a concussion, but has since been discharged from hospital. Lloyd Eugene Curtis Sr., 77 has now sadly died..

Lillian Mae Curtis and Lloyd were married for more than 50 years and have more than a dozen great-grandchildren.  

Elizabethtown Police spokesman Chris Denham said he was shocked by the accident.

'I've never seen anything like this,' Denham said.

'It's certainly very windy out here and I'm certain that did have a factor and was involved in this.'

'I love you so much nanny. I just want to hug you again and smell your cologne and tell you I love you. How do I say goodbye to you I wish it would of been me in that backseat not you. I just want you back.'

Mrs Curtis's granddaughter, Amy Nichols has launched a GoFundMe to raise money for her grandmother's memorial service. Denny's said it was aware of the incident and was working with the authorities to understand what caused the accident. 'Safety is our top priority, and we are working with the authorities to better understand what led to this situation. Our thoughts are with all of those involved.'
